No risks seen with flu shot in early pregnancy

NEW YORK (Reuters Health) - Adding to evidence that the flu shot is safe for pregnant women, a new study finds no link between the vaccine and the risk of serious birth defects.

The study, of nearly 9,000 pregnant women who got the flu shot, found that about 2 percent had a baby with a major birth defect, such as a malformation in the heart or a cleft lip.

That was identical to the rate among almost 77,000 pregnant women who did not get the vaccine.

What's more, researchers found, women who got vaccinated were less likely to suffer a stillbirth (a pregnancy loss after the 20th week): 0.3 percent did, versus 0.6 percent of unvaccinated women.

Their newborns also had a lower death rate: 0.2 percent died soon after birth, compared with 0.4 percent of babies born to unvaccinated moms.

It's not clear if the flu vaccine deserves the credit. But Dr. Jeanne S. Sheffield, the lead researcher on the work, said it's possible the vaccine helped by preventing severe cases of the flu.

"Can we say for sure that it's the vaccine? No," said Sheffield, a professor of obstetrics and gynecology at the University of Texas Southwestern Medical Center in Dallas.

But, she added, these findings suggest that the flu shot is at least safe, and possibly has a benefit against stillbirth.

Sheffield and her colleagues report the findings in the journal Obstetrics & Gynecology.

The Centers for Disease Control and Prevention (CDC) and other groups recommend that all pregnant women get a seasonal flu shot.

That's because pregnant women are more likely than other women their age to get a severe case of the flu or have complications, such as pneumonia. The flu is also thought to raise the risk of preterm delivery and fetal distress.

Still, Sheffield said, many women and doctors alike still have concerns about flu vaccine safety during pregnancy - especially the first trimester, which is when birth defects form and when most miscarriages happen.

But in this study, the researchers found that vaccination during the first trimester came with no increased risk of birth defects.

Despite recommendations to get the flu shot, most pregnant women do not. In the U.S., only between 10 percent and one-quarter of women have been vaccinated each flu season over the last couple decades, Sheffield's team notes.

Based on studies, that seems largely due to safety worries.

On the other hand, Sheffield said "it's amazing" how many women are unaware that the flu itself is considered a risk during pregnancy.

"The flu is a problem in pregnancy," she said. "But we have a vaccine to prevent it. And it's considered safe and effective in any trimester."

A CDC study published last year found "no unusual patterns" of pregnancy complications or newborn health problems among U.S. women who received the flu shot between 1990 and 2009.

Based on cases reported to the CDC's Vaccine Adverse Event Reporting System, the rate of miscarriage was 1.9 per one million vaccinated pregnant women.

Cowboys have guidelines in place for Dez Bryant

IRVING, Texas (AP) ? The Dallas Cowboys are trying to provide some guidance for Dez Bryant, the talented young receiver who has had issues off the field.

Coach Jason Garrett wouldn't go into specifics Monday or describe potential discipline for not abiding by the rules, but he said the Cowboys (No. 15 in AP Pro32) want to support Bryant and his family.

"What we've tried to do is come up with a plan for Dez like we would for any player who we feel like needs our support, and help him be his best as a player and as a person," Garrett said. "And the accountability factor is an important part of that with him and with anybody on our football team."

According to numerous reports, Bryant will attend weekly counseling sessions, have a full-time security team and not be allowed to consume alcohol or attend strip clubs. The security members will drive Bryant to team functions, including practices and games, and the receiver will also be subject to a curfew.

Bryant was arrested in July for allegedly assaulting his mother, who has since said she doesn't want authorities to pursue charges against him. The Dallas County district attorney's office has yet to make a decision about misdemeanor family violence charges.

There is still the chance that the NFL could discipline Bryant, who is going into his third season with the Cowboys.

NFL spokesman Greg Aiello told The Associated Press in an email Monday that the league "will review the facts once the case is resolved by the court." He said the team's guidelines for Bryant will have nothing to do with league discipline.

"We're going to control what we can control as an organization, and Dez has done everything that we've asked him to do up to this point both on and off the football field," Garrett said. "Any decision about that is out of our control."

Despite other off-the-field issues in the past, Bryant was having a solid offseason until his arrest two weeks before the start of training camp. Bryant's adviser, David Wells, and the player's attorney, Royce West, didn't return messages to the AP.

"We have some things in place and we are still in the process of working out the rest," Wells told the Fort Worth Star-Telegram in a story published Monday. "We are all working together to accomplish some things to help him succeed."

Bryant hasn't been available to speak to reporters since the start of training camp because of his pending legal issues. He didn't talk in the locker room after practice Monday.

In 2008, the Cowboys had a security detail in place for Adam "Pacman" Jones, who had missed all of the previous season with Tennessee and was coming off a 17-month NFL suspension because of repeated legal troubles. Six weeks after being reinstated by the NFL, Jones was involved in an alcohol-related scuffle with one of his bodyguards at a private party in Dallas and was suspended for six games. He was cut by the Cowboys after the season.

Cowboys owner Jerry Jones waited a couple of weeks after Bryant was arrested, and a couple of days after the team got to California for training camp, to talk with Bryant. The owner, who called it a serious situation, said he was disappointed and didn't want to be emotional when talking to Bryant.

Jones said then that Bryant had already been involved in the team's counseling program.

Bryant surrendered to authorities July 16, two days after his mother, Angela, made a 911 call about the alleged assault. The 23-year-old receiver is only 14 years younger than she is.

There had been some trouble for Bryant even before the Cowboys drafted Bryant with the 24th overall pick in 2010.

Bryant missed nearly his entire last college season at Oklahoma State because of an NCAA suspension for lying about having dinner with Deion Sanders. He ran up hundreds of thousands of dollars in bills on game tickets and jewelry, and was sued by people who said they were creditors.

Last year, he was kicked out of an upscale Dallas mall for wearing sagging pants. In January, he was reportedly involved in a fight with the rapper Lil Wayne at a Miami nightclub.

Bryant said during the offseason that he was spending more time with the playbook, watching more film and concentrating on his conditioning. There were no issues until the incident with his mother, and haven't been any known problems since then.

During the early portion of practice Monday open to reporters, Bryant was initially catching some passes from backup quarterback Rudy Carpenter. Bryant, who has been dealing with right knee tendinitis, then did some rehab work with a trainer alongside tight end Jason Witten (spleen).

Bryant and the most of the Cowboys starters aren't expected to play in Wednesday night's preseason finale against Miami. That comes a week before their regular-season opener at the defending Super Bowl champion New York Giants.

Teammate Marcus Spears said the guidelines were between Bryant and team ownership, but that he was "pretty sure" they were on the same page.

"Whatever they're doing, they're doing the right things," backup quarterback Kyle Orton said. "He's a special player. He's one of those guys, you find ways to get him the ball. ... You want to give him a lot of touches because one of those touches is going to be a special play, and there's very few of those guys around."

As Isaac draws near, families are making sure everyone is safe?including their pets. Pets enrich the lives of individuals and families in more ways than can be counted.??In turn, they depend on their owners for their safety and well being. ?Just as households prepare to take care of family members; it?s important for people with pets or service animals to take steps ahead of time to keep them as safe as possible during a disaster or other emergency.? said Sandra Hodge, CEO, Mississippi Region. The?American Red Cross?offers the following tips to help people with pets and service animals prepare for a disaster:

  • Assemble a portable emergency preparedness kit for pets. Store items in a sturdy container that can be carried easily (plastic bin, duffle bag), and make sure to include:
  • Sturdy leashes, harnesses and/or carriers to transport pets and service animals safely and ensure they can?t escape.
  • Water, food, bowls, cat litter/pan and manual can opener.
  • Medications, medical records, first aid kit, and veterinarian?s contact information.
  • Current photos of pets in case they get lost.
  • Pet beds and toys, if space permits.
  • Research locations that could shelter pets in the event evacuations are called for.??American Red Cross disaster shelters cannot accept pets because of health and safety regulations. Service animals that assist people with disabilities are the?only?animals allowed in Red Cross shelters. It may be difficult to find shelter for animals in the midst of a disaster, so plan ahead.
  • Contact regional hotels and motels to check policies on accepting pets and restrictions on number, size, and species. Ask if ?no pet? policies could be waived in an emergency.
  • Ask friends, relatives, or others in neighboring areas whether they could shelter pets.
  • Ask local animal shelters or veterinarians if they provide emergency shelter or foster care for pets in a disaster. Animal shelters are caring for the animals they already have so this should be a last resort.
  • Keep a list of ?pet friendly? places that are located along evacuation routes, including phone numbers.
  • If told to evacuate, take your pets with you. If it is not safe for you to remain at home, it is not safe for your pets.
  • If an advanced warning is issued, call ahead to confirm emergency shelter arrangements for pets. Bring all pets inside in case of sudden evacuation orders.
  • Make sure each animals? vaccinations are up to date and that each is wearing a securely fastened collar with contact information. After evacuating, add the temporary shelter location on the back of the pet?s ID tag.?
  • Birds should be transported in a secure travel cage or carrier. During warm weather, carry a plant mister to mist the birds? feathers periodically. Do not put water inside the carrier during transport. Provide a few slices of fresh fruits and vegetables with high water content. Have photo identification and leg bands. If the carrier does not have a perch, line it with paper towels and change them frequently. Try to keep the carrier in a quiet area. Do not let the birds out of the case or carrier.
  • Small mammals (hamsters, gerbils, etc.) should be transported in secure carriers suitable for maintaining the animals while sheltered. Take bedding materials, food bowls, and water bottles.
  • Evacuations of large animals, such as horses or cattle, should be planned as early as possible. It may be difficult to maneuver large animal transport vehicles in evacuation traffic.

Patients seek medical care to maintain or improve their health, but what happens when a physician makes matters worse? Medical negligence is difficult to prove, and it can become overwhelming to victims who are suffering as a result of the negligence. What is it and what are the first steps to take if you suspect negligence?

What is Medical Negligence?

Also called ?clinical negligence,? medical negligence is a breach of a legal duty of care owed by healthcare providers to their patients which results in harm to the patient.

A medical negligence claim is a civil process where a patient pursues compensation for harm that results from a healthcare provider?s negligence. In order for a patient to prevail in a medical negligence claim in the United Kingdom, the patient must prove:

  • The physician had a duty of care to the patient
  • The physician was negligent
  • The patient suffered harm as a result

The patient must prove both liability/breach of duty (the provider acted in a way other competent providers would not have acted) and causation (harm resulted that would not have occurred otherwise).

Medical negligence can occur in any healthcare field including primary care, emergency care, surgical care, dental care, counselling or therapy, and more. Not only can physicians be held responsible for medical negligence, related healthcare attendants such as nurses and pharmacists have a duty of care. NHS trusts, primary care trusts, and private hospitals may also be held liable for the negligence of their medical staff.

Examples of Medical Negligence:?

  1. Failing to diagnose a patient?s condition ? For example, if a patient?s symptoms are overlooked or dismissed as being inconsequential, this failure to diagnose could be construed as medical negligence under the ?Bolam test? if the true diagnosis is likely to have been recognized by another ordinary, competent physicians and harm resulted.
  2. Making the wrong diagnosis ? This is similar to the above. In addition, a wrong diagnosis could cause further harm in that inappropriate treatment may cause further harm.
  3. Prescribing or giving a patient the wrong medication ? This can occur with misdiagnoses or by mistake. For example, if a condition is misdiagnosed, the treatment that follows is based on the wrong diagnosis. What if the physician writes down the wrong drug name by mistake, such as prescribing XYZ instead of ZYX? What if the pharmacist misreads the instructions and dispenses YXZ? These are all examples of potential medical negligence. However, remember that the patient must suffer harm as a result of the negligence. If the patient catches the error before ingesting the wrong medicine and no harm results, then a medical negligence claim cannot be fully established.
  4. The loss of a ?chance? ? In cases where a condition has a high likelihood of recovery, such as 50 per cent when discovered promptly, doctors could be held liable if the condition is not diagnosed because of the loss of a chance to recover (House of Lords 2002 Gregg v. Scott).
  5. Failing to obtain consent to treatment ? In general, doctors must obtain consent before treating a patient. If the doctor fails to do so and proceeds with a treatment that causes harm to the patient, the patient may have a legitimate medical negligence claim.
  6. Failing to warn the patient about a treatment?s risks ? Similarly, the physician has a duty to warn the patient of the risks of a treatment. Simply asking the patient to sign a consent form is not the same as discussing the risks involved.
  7. Making a mistake during an operation or procedure ? Imagine going into surgery to have your left foot amputated due to an uncontrollable infection and waking up to find out that the surgeon amputated your healthy right foot by mistake. Medical mistakes during treatments and procedures are a prime example of medical negligence.?

The First Steps

According to Patient.co.uk, if you suspect medical negligence in England and Wales, the Civil Rules of Procedure strictly apply (they do not apply in Scotland and Northern Ireland). The first step involves getting legal representation and filing a letter of claim. The defence is allowed a few weeks to gather evidence and respond. Next, the ?particulars of claim? and ?particulars of negligence? are filed and the other side presents its formal defence. Most of this occurs out of court.

Depending on the particulars of the case, it may wind up in court. However, the Medical Protection Society reports that fewer than 2 per cent of claims go to trial. Regardless, the first step is to obtain legal representation.

(AP) ? Typhoon Tembin, which drenched southern Taiwan last week before going out to sea, appeared to be looping back Monday for another run at the island.

The revisit comes after another storm about 1,200 miles (750 miles) to the northeast, Typhoon Bolaven, lashed the Japanese island of Okinawa on Sunday night. It injured five people and left 66,500 households without power as of Monday afternoon, but did less damage than feared before moving north into the East China Sea.

Bolaven could affect coastal areas of South Korea by Tuesday, weather officials said.

Taiwan's Central Weather Bureau predicts that Tembin will make landfall early Tuesday in the same part of southern Taiwan where it dumped more than 500 millimeters (20 inches) of rain three days ago.

Tembin, packing winds of 119 kph (75 mph), would likely skirt the eastern Taiwanese coast before moving northward toward the Chinese mainland, the bureau said.

Disaster officials in Okinawa were relieved that Bolaven, which was billed to be the strongest storm to hit the southern Japanese islands in several years, ended up being weaker than expected.

Okinawa authorities reported no major damage Monday aside from the blackouts. Officials in the nearby Amami islands said they had reports that some houses were damaged, but information was still being collected.

Many schools and government offices were closed because of the blackouts.

Buses were not running Monday, but Naha Airport, serving Okinawa's capital, began functioning again, with 89 domestic flights operating and about 30 cancelled.

It wasn't immediately clear if ferry service had restarted amid still rough seas.

As Bolvaen, the 15th storm of the season, approached Okinawa on Sunday, the Japan Meteorological Agency said wind speeds near the center of the typhoon were about 180 kph (112 mph), with gusts reaching 252 kph (155 mph), possibly equaling or surpassing past records for the area.

But public broadcaster NHK reported that the gusts measured on Amami island north of Okinawa, reached just 140 kph (87 mph).

Okinawa disaster authorities said five people were hurt, including one 75-year-old woman who broke her hip when winds knocked her over.

As of Monday afternoon, 56,700 households were without electricity on Amami island, while 9,800 households on Okinawa were still lacking power.
